Somatic symptoms are as popular as emotional symptoms in depressed patients. However, clinicians have underestimated somatic symptoms in the diagnosis and treatment of depression. Somatic symptoms frequently occur under stressful life events of depressed patients. Fatigue remains as a residual symptom in depression and it may be a burden on the treatment of depression. There are some results to the efficacy of antidepressants combined with psychostimulants or modafinil for fatigue symptom of depression. When depression is combined with physical illness, the somatic symptoms can be worsened in the course of both illness. In these situations, clinicians need systematic and comprehensive approach for careful medication selection. SNRIs are more effective than SSRIs in the treatment of somatic symptoms of depressed patients, and duloxetine is effective in chronic pains of depression. NDRI can be used for psychomotor retardation, and may be considered cognitive therapy with pharmacotherapy. Somatic symptoms are associated with adverse effects of medication, poor prognosis of depression and treatment resistant depression.
